Any PSD provided is simply a bonus, so if you start designing and finish your design in HTML (like I often do), you can just include a PSD of whatever you have and warn potential buyers that a full PSD is not included.

First: Its subjective. You will have to find your own preferences

If you want my opinion: netbeans for everything php related (a real ide with useful code completition), textmate for quick changes and small projects (like html only stuff)
