I’m getting lower number of visits, maybe it’s because of new Google changes. On the other side, I noticed that larger ammount of users are visiting my item from search page (on CodeCanyon).

This is very good thing, but design is poor. I can’t find which font you are using so I assumed you didn’t put any font, try to create some custom radio buttons (in menu on the left) instead of using default one. Just improve design and I’m sure it will be approved.

I believe you should add some data validation, for example I can put anything or even leave the input fields blank and still I can go to next step.

You are absolutely right, one of my items have 4400 visits this month, and I had 40 sales of that item, similar with other items.
After best 27 days this month, sales stopped for 2 days
