I want a Simple landing page with form plus add to cart options.

This could be in html/html5/xhtml.

The form would have drop down and check boxes to choose and then the calculate and add to cart button below that.

Example : [checkbox] : service 1 [checkbox] : service 2 Choose options : drop down:big pack[drop down:small package]

Is there something like this available on themeforest ?
