My first item was soft-rejected due to poor documentation. And I think that’s one of the main reasons why items get soft-rejected. I went back and improved the documentation. Since then I’ve put some extra work on doing a better docs for the new items I submitted.
I believe Reviewers could have even higher standards for it. It would just be good for the marketplace and for the authors themselves.
Since English is not my native language and I am not very good at writing long texts, I know my documentation is not the best, it could be even better. But I try make it up for it by having the best support possible
I had a similar situation once. Item selling well, 5 star rating appears. Fourth rating is 1 star. Not a single instance of negative feedback.

But I’m confident that when the other buyers start rating (with the rating reminder emails) that 1 star rating will be insignificant for the overall average.

A good author is someone who cares about his buyers.
The hardest part of support for me is when there’s a bug in a customer’s theme, and they blame my plugin for breaking everything. WordPress plugins seem to have an undeserved bad reputation. It’s our job to help change this, to educate people. That’s also a part of self-promotion.

Yeah.. most common problem for me also.
I never had a client refusing me the login details, but I rarely ask. It’s a bit like nCrafts said, I ask them to do some stuff or give me login details when needed. Most of the times they offer to do it.
I guess the trick is not to ask it straight away, try to solve the problem some other way, or ask them to check the stuff you need.
It’s a tricky one.
