There are many ways for a hacker to gain access of your server. What kind of server do you own? If you have a regular shared account, you can get hacked, no matter your application security, if another account is hacked and the server is misscofigured, a hacker can browse the whole www directory. Anyway, the first step in detecting the way you got hacked, is by analysing the apache/php logs.
