I am puzzled on this one.. if I have a purely graphical item with no possibilities of bugs like for example in PS Actions, then what is shown in the preview is exactly same as what is in the file, right?
What is shown in the preview is just a screenshot of what I have on the file, I did not do anything magical with it I just took a little stupid screenshot, then why would someone rate the item poorly after purchase when it is actually exactly as in the preview?
I think it just happens because each buyer has different needs and expectations. Try not to overthink it.
However, if you get a few in a row, take a look at your files and see if you can improve things. We have 3 – 1 star ratings on a products with 34 – 5 stars and 7 – 4 stars. Seeing this it was obvious that for a small group of buyers we were doing something very wrong. We reviewed the file and made some updates to correct the problem.

If you have good items they get noticed, that is all we are saying. Not every item uploaded sells. Every author has an items that sells less than they expected, you just have to keep on pushing forward.
Focusing on quality is the most important thing not only for sales but also for personal development. If you keep pushing on quality and adding 1-2 files every month then over time you will build a great portfolio, your reputation will improve and people will trust your brand and buy your files.
Marketing works to a point, but you can’t sell crap for too long with just clever marketing. Only a naive marketeer would say those things.
Someone mentioned it as a marathon, think of it as a marathon you don’t want to end because you enjoy the running.
