I would say .po files are the way to go because if the buyer needs to have the site in multiple languages, how do you address this using translation in the admin panel?

Kriesis’ code should work, try the following instead to get the featured image attachment ID:

$image_id = get_post_thumbnail_id( $post->ID );

Untested by the way and I’m not on my computer to actually check the code I use for this purpose.
