Hi, I need a template that is modern and allows for forms to be put in. Users will log in and fill in a form to have there item collected by courier. Thanks if you can help.

I think you may be able to look at this in a different way – if you use WP then finding a “modern theme” – well there are hundreds of there and the important form details etc you want – you would need to find a plugin or custom code a form to store their details and send it to the courier company to collect.
I highly doubt you will be able to find a theme template with this functionality already in it and even if you did there are all sorts of security and privacy issues that would need addressing very carefully.
Best bet is find a theme you like and get a freelancer to customise it and the form/delivery element for you.
