I’m thinking of purchasing a Parallax banner for my new WP theme. I was wondering, if I purchase the extended licence I can use it with the theme, right? Also, to be able to use it in more than 1 application, do I have to re-purchase it?

Exactly, EL grants you the rights to include the item in a WP theme for sale on ThemeForest and you will need to purchase one EL per released theme.

Actually I think I have misguided you. The licence terms have been changed few days ago and it looks like EL will no longer grant you the right to include the item in your own item by default. Read more here.
