Hey guys, this is probably easy but just cant seem to get my head around it. I want to have 5 forms in 1 form, and depending on your first answer is what form you go. All forms gather the same information except for a few questions.

Ive got it working on wufoo see below http://testmel.wufoo.com/forms/untitled-form/

So when an option changes it shows hidden questions.

What is the actual problem you are having?

You can do this through the use of Javascript/JQuery and then you just validate it on the server side.
