FORM SIGN UP

Im looking for a Form so that people can sigun up to support or virtually adopt at the NGO where I work.

Im going to translate it to spanish so must be easily editable.

The form will have name, surname, id number, account number, sign up options, yearly monthly, etc.

All this data will be sent to us and we will receive the info in a mail.(is there anyother way to do it?¿)

So Im looking for a good form and help and suggestions!

Thanks for the suggestion. Ill have a look at all sugestions but first Im going to go into my problem a little bit further.

I work at a small NGO , their building a small wordpress website and one of the options they want is for people to be able to join the NGO online.

People can chose to make a donation, or join the NGO and pay a monthly or yearly subscription.

People can either decide to help the NGO generally, and their subscriptions go towards covering the running of the NGO and or the projects that have been taken by the NGO .

Another option is to virtually adopt a child, so people can adopt children in south america, africa and their subscriptions are recieved and sent to the childs family.

They want all this to be able to be done online through a sign up form. (currently its done via paper sign up forms which people fill out and send via post to our main offices, once we receive the forms we put the data into the computer.

Now first of all. Can this sign up form be done online, if so can it be done so that once the info is filled up we receive it as an email?¿

Second is there a better way to do it, or will it get complicated?¿

The info we requiere from the person that wants to sigh up is:

Name Surname Account number Id card number Type of Member: General member Virtual adoption Type of member: Monthly Yearly adress phone number email
