Price would depend on the exact requirements. No one would give you a price here and you didn’t gave us a contact details.

Probably it would take 1-2 hours which means 20-80 USD (depends on author’s hour rate).

For example – mine is 20 euro/hour which means – 27-28 USD /hour.

No one could give you a precise quote or estimation without more info.

As above what features would you like e.g.

capthca, validation, upload form etc etc
