I’m not a WordPress developer, but I’m sick of seeing these threads and I guarantee WP authors are sick of support requests for this.
How can ThemeForest end this problem for good?

One way I really thought of is create a false theme and dropping it in the main download folder with an alert saying you must extract it/ or run a script to extract and install itself
it works but I decided against it because it leaves a few extra files in the main zip which could end up confusing the user just as much when they first unzip, The other issue is if authors have PSD files etc it could just be to large for their servers to upload which causes other issues.
the only other way is if TF would be warning on the download page/ in the download email saying the files must be extracted first, but even then Im not sure it will stop the issue
Why doesn’t Themeforest just name the zip file something like:
themeforest-3102918-itemname-UNZIP-ME.zip
Seems like an easy solution?

I think at this point we need to separate it all out. There should be several options (buttons on the Downloads screen)
1. Download theme (make it a shiny color)
2. Download documentation (or just make it available online)
3. Download full package (includes Theme + Docs + Sample Data + anything else)
This solves two issues: the old “style.css is missing I want a refund!!!” issue and the “where the hell is the documentation?!?” issue.
