Job Application Form For Various Recruitment Agencies

Hello,

I need someone to create a simple job application form that users can enter all their details into including images of themselves (with file limit) and other information and then select which recruitment agency they wish to send it to and submit.

Then in the back end the recruitment agencies can log in and view the individual entries that have been submitted to them. they can then download them and also delete them. To start there will be just two agencies with the plan to add more. The form needs to be able to be edited with new fields from time to time.

That is all for now I also have other work for the right person/team.

I need a very fast turn around on this project of a most two weeks from now.
