Need A Form plugin

I have searched through codecanyon for a plugin that works like the one on this link:

I want to create a form on a page, whereby people can search for list of persons or companies on the website, by using profession, location, and other defined criteria.

I would appreciate if anybody can recommend a plugin for me.
