Need image upload script

I need a image / file upload script with just a few things,

1. login / reg

image upload part

file upload part

Here is a link to what I think the image upload and file upload will look like,

I would like to get an email something like this:

Name: John Doe As just upload a file.

Then have an admin login where I can click there name and view any images or files they uploaded..
