Hi guys, has anyone worked on a flexible online form builder?

Gathering information from a client online instead of word doc! Would be a mix of a form you expect to fill in for Job application/ Contact / Order / Event etc. Needs to be flexible with a mix of drop down menus, free text, radio buttons etc and possibility to add or remove (i.e. enter your email, and add another field for secondary email etc) and re-name each field as we see fit. Not expecting the perfect solution there but if you have something ready we would love to hear about it. Thank you!
