PHP Contact form!

Hey guys im working on my new theme,and i would like to make an working contact form, in php and jquery falidation, since im not too familiar with php, i want to find a source code, but i could not find a good source out there, i had some time ago a script but lost it somewhere, if you have any reasource please share it, it will help me a lot, also if it has jquery validation as well!
