Read the documentatation. Because Envato doesn’t allow font files included, you have to download it, it’s all in the docs.

When the user enters the email address, where is it sent to , and how do you set this up?
Emails are logged on a file called “list.txt”. You need to have an access to FTP account. Upload your files to the server and enjoy!
You should read the documents, there is more information about the settings etc.
If I wanted to add a social media icon that isn’t included in the zip file, how would i do so? I’m not sure where the images are actually stored, as it seems they are embeeded into these font files.
The social icons are embedded as a font. You can replace it with image, I assume you have some basic HTML knowledge how to add image. Just make sure it’s right size to make it look good.
Ok, thank you. Got that one figured out already. The font idea is cool, just didn’t work for what i was doing. I am having some difficulty with this signup form. Is there supposed to be an additional php file with this? I ended up writing my own to store the user info as txt file, but I figured I’d ask because i didn’t see any mention of it in the documentation.
I’m sorry, there is no PHP file included with this template. My 2 newer themes actually have one (works exactly like yours), which I could adapt to this one too. Thanks for the idea!
