Hello! About the form,please be sure that your hosting account has PHP and that mail is allowed on php,contact your host. About the map, all websites registered after June 22nd using the google maps API or any other API will not be able to use the service unless they register a valid API key, Get you API Key here: https://developers.google.com/maps/documentation/javascript/get-api-key
