Contact Forms working added after your purchase, so kindly download the Updated template,
in recent download you will find a file process_forms.php << open that file change Email $mail_to to your Email, and at End replace reply to and email from to a email address which relates to your domain name where that website is working.
Then put that file in your root of site, and use new Forms copying everything including <form> to </form> from the recent download you get. thanks

These icons are included in the Template you can just find the icon you want to use, and copy its class name, and use something like
<i class="fa fa-user" /><!-- fa-user is what you copied -->
