I just bought a license for the Study LMS template and I see there is a file missing from the distribution:

- chosen-sprite.png

It is used as a background image in css/style.css on lines 260, 298, 314, 453.

I have no idea what the image might show, but I think I need a copy.

Your demo site is also missing this file.
