1. Yes you can implement custom HTML to have rows / columns with background image as per your need.

2. Yes you can add 2 columns in a row and then in each row add list element. This is purely DIV based template, so we do not have pure table but you can check pricing tables here – http://www.themezaa.com/html/pofo/pricing-table.html which are implemented using DIV tags.

3. We do not have ready form like mentioned above, so you will need to achieve it using custom HTML and JS as per your need.

Only a single line code, or even only the checkbox. The rest (html and js integragtion I do my self. I have own email system and know how to integrate its code and js into the code line(s). So NO further support with this needed
