Thanks for asking. We created appointment booking form http://labpeak.themetechmount.net/book-appointment/ using paid plugin called Booked and it’s not added in our theme package. You need to purchase it separately. But we also added support for a free booking plugin called Booking calendar, Appointment Booking System. So you have option to use which plugin. You can try the free plugin first and then decide yourself.
