Really nice layout. Remember though on the Form elements you really shouldn’t consider yourself done unless you have implemented Datatypes / Masking for every known datatype you can think of as well as controls like Date / DateTime / Phone / Hex / URL / IP Address / Multi-Select (with search/checkboxes). Something that would really sell this template is to put a human figure on it to where the physician can click on areas of the body and take a note as to a pain or symptom or fracture on each visit. Also think of hospital colors. While the green is nice a light CYAN is a medical uniform color.
