Hello, first be sure that the form point to phpmailer/survey_phpmailer_template.php
You have to edit this file js/survey_func.js and point the form to the correct php file by changing this line
$(‘form#wrapped’).attr(‘action’, ‘survey.php’);
into
$(‘form#wrapped’).attr(‘action’, ‘phpmailer/survey_phpmailer_smpt.php’);
Be sure that your smtp credentials are correct. If the issue persist, you should also contact your host provider support that probably have temporarely disabled the mail functions due to incorrect settings.

Hello! Pre buy question. I really like your form, but i have some advanced things i would like to know if you guys can make. Like storing a person summary in a dashboard. Is this possible for you guys to create for $money of course.
+ There is some more things, but ill have a document for the whole project. I just need to know if you guys are up for the task and of course capable to make it.

Sorry, but I must it to write. It sux. This template is useless if You want to put it in CMS system (not wordpress – dedicated). Why html page with form is loaded to php file with settings instead using normal post method?? I was buy it and I don’t need it. And 25$ is goooone.

Hello, we are sorry not. It is html based. A clean a simple way is to create a subfolder in your website root (with an ftp cllent like Filezilla or you host panel), and upload all the files there…so it will be www.yourdomain.com/wizard

1) The length of time for the form to submit seems excessive to me – is there any way to fix this?
2) Is there any way to integrate this with a scheduler? Is there any you’d recommend? I’m looking to create a form similar to this: https://upside.com.au/appraisal/address (you’ll notice at the end there is an address)3) Is there any way to capture the information from the form if the user chooses to leave early? (e.g. if a user fills in the name, email etc. of step 1 but leaves on step 2.

Hello, thank you. 1) it’s not very clear what you need. you can place your terms inline beside the checkbox without modal or inside a scrollable textarea for example. 2) 755 it’s common for upload folders (same used for wordpress or other cms). it’s quite safe because external user can only read or execute. 777 is dangerous. if you need to protect the view of that folder, usually with a password, this is beyond the scope of thi item (requires php+database to create an access application). you can look for your hosting provider, someone provide via panel a protection folder feature. all the best
If I type domain.com/upload_files i will be able to see all the files and download. You should add an empty index.php file inside this directory.
Hello, thank you for the tips. I will investigate or apply your solution, but seems also that depens how the host is configured because mine (and others) doesn’t show anything. Would be better for sure change the name folder to someone less common. All tne best
