Regarding to your questions:
1) Yes. Google Maps API key is required and needs to be set up on your Google Cloud Console account. Complete instruction how to do this is available in the documentation.
2) Using Google Maps API is paid. Please note, that Google offers $200 free credits per each month what is enough value in almost all cases.
3) It is not possible to switch Google Maps to any other library like e.g: Open Street Map. Only the Google Maps offers all of the features which are needed in this type of plugin.

1) From the customer point of view, the better solution is choosing a car than driver, so – the option which you are looking for – is not available directly in my plugin.
2) Please kindly notice, that you can assign (via the WP dashboard) driver to the vehicle. If you wish, I can modify the plugin (in scope of my freelance job) to display name of the driver next to the car in step #2 of the booking form.

I am only using distance in form on time of one way the calculation working fine but when i select the return doesn’t work well for example Initial 10 Initial per mile 2
so if i have one way of 2 mile = $10 Initial + ( 2 mile = $4 ) so total will be= $14
i have same amount put in Initial return 10 Initial return per mile 2
so if i have return of 2 mile = $10 Initial + $10 Initial return ( 2 mile = $4 ) ( 2 mile return = $4 ) so total will be= $28
But amount shows wrong.
