Yes you are right, it works well in the demo. I try the original content-group.php file but the problem still exists. I guess the problem is caused by going to the wrong url link when clicking submit button.
I will look for the solution in other files.

i am using cloudways plugin elastic email and rocketspace email . i can send custom email but forgot password dont working. cloudways agent told me that i dont need to install wp mail smtp.elaastic email is enough
We are using only the default WordPress function wp_mail so check the email problem with your host.

This is not available, we have 2 menus already in the theme, one in the top and another on the left.

3 – We don’t use any out plugin, all of the code in our theme and our plugin, so we don’t support any option plugin to do that on their forms.
4 – No custom design for it, but it works well.
5 – No, but any plugin will work by default. Only you don’t use any member or login system because maybe it will make conflict.
