you should provide images also ,main look comes by its graphics ,if without graphics then may be only few will go with this theme
At Envato no rules for giving original graphics or images – because some images we used for demo purposes. Customers can use those by buying the license. If still want your graphics try to send your ftp access and change the rating to 5 star. So that we can upload into your server. Thanks

I’ve already done it. Thanks. But I’d need it to be customized because it’s just showing on top of the form.
I think it’d be better to write a code probably in PHP or JavaScript to make it look nicer. Probably the form button should roll and show the m message on the screen probably removing the form and making it the message displayed.

The template is not available. Themes need an index.php template file. Child themes need a template header in the style.css style sheet.
