This is a the shortcode for “Plan A visit” form. If you are using the layerslider at that section, just mention the slider id value only the number and the slider as “LayerSlider” as shown below. Else if you are using revolution slider just use name inside the bracket for ex: About Slider (about), the slider id that should be entered is “about.”
