Please note that it is possible to use only one Footer Top area through the website, but you can turn off Footer Top and Footer Bottom and use Content Bottom Widget Area instead on some pages:

Hi Question – is the contact form placed above a slider revolution? Would love to have contact forms on themes for digital agencies. Conversion (contact forms) is very important for us also. Definitely can make some tweaks to fit in the HouseMed theme to a digital agency though.

Here – http://housemed.mikado-themes.com/dental-clinic/ – Is the “book an appointment” contact form there placed above a slider? I noticed that the background images are sliding. I use slider revolution on my theme (and many of mikados theme use slider revolution also since I’m a big fan). If I am going to get the housemed theme, I want to know if calling the “book an appointment” contact form is easy and can be placed above slider revolution.
