Contact Form 7 has some Datepicker support via the new HTML5 date input type ( checkout – http://contactform7.com/date-field/ ), but currently this option supports only on few browsers -http://www.wufoo.com/html5/types/4-date.html

So please Contact Form 7 Datepicker plugin. Install Contact Form 7 Datepicker plugin and use date field as usual in the Contact Form 7 Plugin. It will display date picker in the date field.

I ‘m afraid that “Contact form 7” Plugin is only for making contact forms.

I need a plugin for the users to register (something like membership) so I can provide them special access to some content. For example a patient’s personal medical history.

Wordpress offers such a thing (www.example.com/wp-login.php) but the interface is not professional and the most important, there is not an easy way to embed it in a web page …
